Martin, a 44-year-old male, has suffered from ALS, accompanied
with the inactive walking and loss of his balance of the two
lower limbs for a year and a half. He was hospitalized in our
hospital at 20:00 on Apr. 2, 2009.
Essentials for Diagnosis:
1. The patient has
suffered from ALS, accompanied by the inactive walking and loss
of his balance of the two lower limbs for a year and a half.
2. In August, 2007,
the patient began to feel the feet
prolapsing
without obvious reasons.
He felt
weakness of the
lower limbs especially the right lateral position.
The muscles of the four limbs occasionally twitched, and that of
the two legs twitched more obvious. So he went to the local
hospital (name unknown). After the inspection of CT, he was
firstly diagnosed as ALS. He
didn't take any
treatment apart from taking Rilutek capsules. So he came to our
hospital on Apr. 2, 2009 for treatment.

P 80 times/minute, R 20 times/minute, BP 130/70mmHg.
4. He grew normally
with medium-level nutrition. His mind was clear. He had an
expression of chronic illness and tiredness. His body was
cooperative with his mind.
5. No thoracic
deformity. The sound of breath was bilaterally normal on
auscultation. No sound of pleural friction. The heart border was
not enlarged. The heart beat was 80 times/minute. The cardiac
rhythm was regular. No pathological murmurs on auscultation.
6. The
atrophy
of
the right thigh was obvious. The right feet
prolapsed.
He couldn't walk very smoothly.
The muscles of the four limbs occasionally twitched. The
myodynamia of the two lower limbs was of Grade¢òwith
reduced
muscular tension.

Diagnostic Basis:
TCM:
The patient's two lower limbs are inactive. He has lost his
balance for a year and a half.
He has
spleen-stomach vacuity. Lack of qi and blood makes it unable to
supplement nutrition to the body and sinews. So the body is
weak, and then the
atrophy worsens gradually. The waist is the house of the
kidneys, the kidneys govern the bones. Lack of the essence
causes the limp waist and knees as well as emaciation of the
muscles, which results in wilting pattern.
Western Medicine:
The patient's two lower limbs are inactive. He has lost his
balance for a year and a half. The
atrophy
of
the right thigh was obvious. The right foot
prolapsed.
He was unable to walk very smoothly.
The muscles of the four limbs occasionally twitched. The
myodynamia of the two lower limbs was of Grade¢òwith
reduced
muscular tension.
Diagnostic Differentiation:
TCM:
The patient's wilting pattern should be differentiated from
impediment pattern. Wilting pattern is mainly characterized by
limp, weak, and emaciated limbs with the numbness of the
muscles. But the patient usually has no joint pains. On the
contrary, impediment pattern is generally characterized by the
joint pains. So they are not difficult to be distinguished.
Western Medicine: ALS should be differentiated from tumor of
spinal cord, which is characterized by nerve root pains of the
upper limbs, but without extensive trembling of the muscles.
Besides, there is disturbance of sensation and obstruction of
the subarachnoid cavity. The disease can be diagnosed clearly by
the examination of CT or MRI.

qi and blood .
Western Medicine diagnosis: ALS

Then he has taken Rilutek capsules up to now. Presently the
patient began to feel

He felt
weakness of the lower limbs. His sleeping was normal. His
appetite was ok. His urine and stool were regular. After the
inspection, the patient's atrophy of the right thigh was
obvious. The right foot
prolapsed. He had floating pulse in the left wrist. His tongue
body is thin and fissured. He had tooth-marks on the edges of
the tongue.
According to the symptoms of the tongue and pulse, Professor
Yang believed that the patient had suffered from wilting pattern
due to the spleen-kidneys vacuity and obstruction of qi and
blood. So the treatment strategy for him was to fortify the
spleen and boost the kidneys, move qi and quicken the blood.
The prescription was as follows: baizhu (ovate
atractylodes root), chuanxiong (ligusticum root), etc. 4 dosages
in total.

The patient said that the muscular jumping of the lower limbs
lessened, but that of upper limbs occurred again. The numbness
